About the Project

Patients’ medical records are held confidential as well as an important resource for future patient consultation. Conventionally, patients’ medical records are recorded on paper and stored in physical storage. This method is inefficient and outdated. Records are prone to errors, can be misplaced, and are very time-consuming to retrieve. The pre-existing system of records management will slow the response time given by hospitals to the patient’s needs and inquiries. There is an intensified need to address this issue and further improve records management operations in hospitals. Technological innovation has empowered the healthcare field and provided them with better tools that will ease up their operation. The capstone project, entitled “Online Medical Record System” is designed to automate the recording, storing, and retrieval of patients’ medical records. The system is a better initiative that will ensure accurate and sake-keeping of medical records.

Objectives of the Study
1. To allow electronic recording of patients’ medical records.
2. To store medical records in a centralized platform.
3. To increase the accuracy of the information recorded in the patient’s medical history.
4. To strengthen security and confidentiality of patients’ medical records allowing only authorized personnel to access them.
5. To allow easy and fast retrieval of patients’ medical records.
6. To evaluate the system in terms of user acceptability, efficiency, portability, productivity, quality, and reliability.

Input

User Requirement –This is the area where the researchers obtained information about the system’s unique properties. It is vital to consider their suggestions because they will be the ones using the Online Medical Record System.

Review of Related Literature – The researchers have conducted investigations and studies in the proposed system’s region. This procedure will be useful to the researchers as they develop the project or system.

This is where the researchers can now use their knowledge in programming to design and develop the system.

Process

Data gathering

During this stage, the researcher for the study gathered all potential system development demands, including those obtained from the system’s beneficiaries, the end users. The researcher created the survey questionnaire, which was then validated by professionals. The surveys were created to collect information to improve the performance of the proposed system for future community use.

Data analysis

Consultation is used to obtain end-user requirements and generate ideas. We also distributed a survey questionnaire, which was approved by all three experts (IT Expert, English Grammarian, and Researcher). These questionnaires also provided a data collection tool for assessing the performance of the manual system, which served as the foundation for developing our proposed system.

System Design

The prototype and planned system features are developed at this phase. A concrete understanding of how the system will function is also created. In this section, we determine all of the relevant system inputs and outputs, as well as the data, process, and interface designs.

Programming

In this stage, we materialized the whole idea of the software to be designed. We created the program for the proposed system. The actual coding of the software is based on the system design and the requirements that need to be met. This is where the proper execution of the previous stages ensures a smooth implementation.

Program testing

In this phase, the researcher performed a series of testing to check for any possible problems that may arise during the implementation and operation of the software and if the specification has been met.

System Acceptance

This is the final stage, where the system is installed and to be maintained after actual implementation. One must take into consideration the hardware and software requirements for the proper installation of the system. As part of the acceptance of this phase, the client is required to have user training to enable them to familiarize themselves fully with the whole system.

Output

The project comes to life and is carried out in the real world once all of the necessary procedures have been finished. A new project is born, and it will be maintained for the project’s long-term survival. The Online Medical Record System will be implemented and used. At this point, they will be installed, used, and maintained to last a long time.

With the use of an online medical record system, patients can monitor their health records. They may track their visits and drugs with the help of this system, which also allows them to check their health. Additionally, this technology enables people to communicate with their doctors about their health. Patients can access their medical records from any computer or mobile device with the use of an online medical record system. Additionally, this system aids in coordinating medical care between patients and their providers. To provide better care, doctors can easily access the medical records of their patients. The cost of medical care is decreased for both patients and doctors thanks to online medical record systems. By avoiding unneeded doctor visits and prescriptions, patients can save money. By minimizing redundant testing and office visits, doctors can save money. In general, the use of an online medical record system is a valuable tool for improving the management of patient and physician health information.
